Uncomfortable with ordering products on-line?   Uncomfortable with using your credit card on the internet?

Then you can use these three easy steps to print a copy of our mail order form.  Just complete the form (instructions included) using the information you find in the product description pages in our on-line store.  When completed, simply mail or fax your order to us. With the following steps, the order form will be displayed as an Adobe® Acrobat® file.  You will then be able to print the order form, including instructions and shipping rates, right from your computer. It’s easy!!

Step 1. You must have the Adobe® Acrobat® reader installed in your web browser.   If you need to install this free component, you can do so from www.adobe.com.

Step 2. To display the order form, click here, or click on the “Display Mail Order Form” button at the bottom of this page.  When the order form appears in a new browser window, click on the printer icon located in the window’s tool bar or select your browser's File | Print menu options.

Step 3. After you print a copy of the order form (three pages), close the browser window and return to this page by clicking the "X" in the upper right-hand corner of the browser window.
